Excursion

An A330, with 206 passengers and 11 crew, approached its destination in bad weather. There was fog and visibility was low. They aborted the first approach and went around for a second approach. On the second approach they touched down but skidded off the side of the runway and stopped 40 metres to the left of it The crew started an evacuation of the plane immediately and a few people received minor injuries during the evacuation process.

Cargo fire

Here’s the text to the listening from yesterday.

An MD-11 freighter, with 4 crew members on board was en route to its destination when the crew received an indication of a cargo fire. The fire suppression system was automatically activated and for safety reasons the crew decided to divert. They made a safe landing 21 minutes later and evacuated via the emergency slides. During this evacuation, one member of the crew was injured and was taken to a hospital.

Laser beam

A flight was approaching its destination when the pilots were distracted by a laser beam from an unknown source. The pilots tried to continue the landing but one of the pilots was hit in the eye by the laser beam, which caused an eye injury to that pilot. The aircraft was able to land normally, but the injured pilot went to hospital for treatment.

Undercarriage fire

The left main gear of a commercial aircraft caught fire after landing on runway 06 at Schipol International Airport, Netherlands. The fire was extinguished and there were no injuries, but the runway was closed for 45 minutes.

An investigation revealed that the automatic anti-skid system was disabled on the left main gear the day before the accident.

The ground staff forgot to put a warning sticker in the flight deck about the problem with the anti-skid system.

After landing the crew applied brakes, which was in violation of the prescribed procedures when landing with a disabled anti-skid system. The tires burst and a fire started.

Fire in the cockpit

Here’s the text to the listening from yesterday.

A Boeing 747 freighter took off at 11:00am.  15 minutes later the crew declared an emergency and requested a return to their airport of origin because of smoke and a fire in the cockpit. They were unable to maintain altitude. The ATC issued a clearance when the airplane was about 25km from touchdown. The airplane however was too high and passed above the airfield and entered a right-hand turn. The crew was advised that all runways were available for landing. The airplane flew southwest and it lost altitude rapidly.  It crashed into an unpopulated area moments later. The cockpit voice recorder and the flight data recorder were recovered 6 hours after impact and were in good condition.

Burst tyre

Here’s the text to the listening from yesterday.

A Boeing 737-800, taxied from the apron to the runway, lined up and began to take off as normal. As it lifted off the runway a tyre burst. The crew stopped their climb at FL090 and returned to the airport for a safe landing about 20 minutes after departure. After landing the plane was able to stop normally with only a little smoke. Then it taxied to the apron to disembark the passengers.
